Malabsorption and Hair Loss: Are You Absorbing the Nutrients Your Hair Needs?

You can eat the right foods.

You can take high-quality supplements.

You can prioritize protein, healthy fats, iron, vitamins, and minerals.

But there is another part of the equation that is often overlooked:

Can your body actually absorb and use those nutrients?

When I evaluate someone's nutrition as part of a hair consultation, I'm not only interested in what they're consuming. I'm also interested in whether there are signs that their nutritional status isn't improving despite what they're eating or supplementing.

This is where malabsorption becomes important.

Malabsorption refers to impaired absorption of nutrients through the gastrointestinal tract. Depending on the underlying cause, it can affect vitamins, minerals, fats, proteins, or carbohydrates.

And because hair follicles have substantial nutritional requirements, deficiencies caused by malabsorption may eventually show up as increased shedding, poor growth, or changes in hair quality.

What Is Malabsorption?

Digestion and absorption are two different processes.

First, your digestive system has to break food down into components the body can use.

Protein is broken down into amino acids and peptides.

Fats are broken down into fatty acids.

Carbohydrates are broken down into simple sugars.

Vitamins and minerals must also be released from food and made available for absorption.

These nutrients then cross the intestinal lining and enter the bloodstream or lymphatic system so they can be transported throughout the body.

Malabsorption occurs when something interferes with that process.

The result can be a nutritional deficiency—even when someone appears to be consuming enough of that nutrient.

Why Malabsorption Can Affect Your Hair

Hair follicles are metabolically active structures.

During the active growth phase, follicle cells divide rapidly to continuously produce a new hair fiber.

That process requires a consistent supply of raw materials.

Some of the nutrients particularly relevant to healthy hair include:

  • protein and amino acids

  • iron

  • vitamin B12

  • vitamin D

  • zinc

  • essential fatty acids

  • other vitamins and minerals

When nutrient availability becomes inadequate, the body prioritizes essential organs and biological functions.

Growing hair is not essential for survival.

That means the follicle may be one of the places where nutritional insufficiency eventually becomes visible.

The Difference Between Eating a Nutrient and Absorbing It

This distinction is incredibly important.

Let's say two people eat exactly the same iron-rich meal.

One person may absorb and utilize the iron normally.

The other may have a gastrointestinal condition that interferes with absorption.

Their dietary intake looks identical.

Their nutritional status may not.

The same principle can apply to vitamin B12, fats, protein, and other nutrients.

This is why I don't assume that someone has adequate nutritional status simply because they tell me they eat well.

Sometimes blood work tells a different story.

Iron and Ferritin

Iron is one of the most important nutrients to consider when discussing hair shedding.

Ferritin reflects the body's stored iron reserves.

Low ferritin is commonly evaluated in individuals experiencing diffuse hair shedding, particularly women.

Iron absorption primarily occurs in the small intestine, and several gastrointestinal factors can influence how much iron ultimately becomes available to the body.

When ferritin remains persistently low despite adequate dietary iron or appropriate supplementation, it's worth investigating why.

Possible explanations include:

  • ongoing blood loss

  • heavy menstrual bleeding

  • inadequate intake

  • increased physiological demands

  • impaired absorption

Simply increasing the supplement dose without identifying the cause isn't always the best solution.

Vitamin B12

Vitamin B12 absorption is even more complex.

Before B12 can be absorbed, it has to interact with several components of the digestive system, including stomach acid and intrinsic factor, a protein produced in the stomach.

The vitamin is ultimately absorbed in the final portion of the small intestine.

Conditions that interfere with this process can lead to deficiency even when someone consumes B12-containing foods.

Because vitamin B12 contributes to DNA synthesis, red blood cell formation, and normal cellular division, inadequate levels can affect rapidly growing tissues throughout the body.

Vitamin D and Fat Absorption

Vitamin D is a fat-soluble vitamin.

That means normal fat digestion and absorption play a role in how efficiently dietary vitamin D is absorbed.

Conditions that significantly interfere with fat absorption can therefore contribute to deficiencies in fat-soluble vitamins.

Vitamin D receptors are present within hair follicles, and vitamin D plays a role in normal follicular biology.

Again, low vitamin D doesn't automatically mean someone will lose their hair.

But deficiency is one potential factor worth identifying when we're looking at the complete nutritional picture.

Zinc and Hair Health

Zinc supports:

  • cellular division

  • protein synthesis

  • immune function

  • tissue repair

These processes are highly relevant to rapidly growing hair follicles.

Certain gastrointestinal conditions can interfere with zinc absorption, while severe or prolonged dietary restriction may also reduce intake.

This is another reason why nutritional deficiencies should be evaluated together rather than individually.

Protein Absorption Matters Too

We talk a lot about protein for hair growth—and for good reason.

Hair is primarily composed of keratin, a protein.

But dietary protein must first be digested into amino acids before the body can use those amino acids to build new proteins.

Most people with healthy gastrointestinal function digest protein efficiently.

However, certain medical conditions can interfere with digestion, absorption, appetite, or overall protein intake.

The result can be inadequate protein availability even when hair is one of the person's primary concerns.

What Conditions Can Cause Malabsorption?

Malabsorption isn't a diagnosis someone should make based on their hair.

There are many possible medical causes, including:

Celiac Disease

Celiac disease damages the lining of the small intestine in response to gluten exposure.

This can interfere with absorption of iron and other nutrients.

Crohn's Disease

Chronic inflammation within the gastrointestinal tract may affect nutrient absorption depending on which areas are involved.

Other Inflammatory Bowel Diseases

Inflammation, reduced dietary intake, blood loss, and altered absorption can all affect nutritional status.

Certain Gastrointestinal Surgeries

Procedures that alter the stomach or intestine can change how nutrients are digested and absorbed.

Pancreatic or Digestive Disorders

Certain conditions can interfere with the enzymes required to properly digest food.

These are medical conditions that require evaluation and management by an appropriate healthcare provider.

One of the Biggest Clues: Deficiencies That Keep Coming Back

This is something I pay particular attention to.

Maybe you've been taking iron for months, but your ferritin remains low.

Maybe you're supplementing B12, but your levels repeatedly decline.

Maybe multiple nutritional deficiencies keep appearing despite what seems like a very healthy diet.

That pattern deserves attention.

It doesn't automatically mean malabsorption is the cause.

But it does tell us that simply adding more supplements may not answer the underlying question.

Why aren't the levels improving?

That's a conversation worth having with your healthcare provider.

Can Malabsorption Trigger Telogen Effluvium?

Potentially.

Telogen effluvium is a form of excessive shedding that can occur after a significant physiological stressor.

Nutritional deficiencies, significant weight loss, illness, and other physical stressors may disrupt the normal hair cycle.

A larger-than-usual percentage of follicles can transition out of the active growth phase.

The frustrating part is that the shedding is delayed.

You may not notice excessive hair fall until approximately two to three months after the original trigger.

That delay can make identifying the cause challenging.

Why Taking More Hair Vitamins Isn't Always the Answer

This is one of the biggest problems I see in the hair wellness space.

Someone experiences shedding.

They buy a hair vitamin.

The shedding continues.

They add iron.

Then collagen.

Then vitamin D.

Then B12.

Then another hair supplement.

Eventually they're taking ten different products without knowing whether they're deficient in any of them—or whether they're absorbing them appropriately.

More isn't always better.

And excessive intake of certain vitamins and minerals can be harmful.

A more useful approach is to understand what the body actually needs.

Blood Work Can Provide Important Clues

When appropriate, I encourage clients to discuss comprehensive blood work with their physician.

Depending on the individual, relevant markers may include:

  • ferritin

  • iron studies

  • vitamin B12

  • vitamin D

  • zinc

  • CBC

  • comprehensive metabolic panel

  • thyroid markers

Blood work can't diagnose every cause of hair loss or malabsorption.

But it can reveal patterns that warrant further investigation.

What I Commonly See During Consultations

Some of the clients who surprise me most are the ones who appear to have incredibly healthy lifestyles.

They eat organic food.

They take supplements.

They prioritize wellness.

Yet their ferritin remains low, their B12 isn't where we'd expect it to be, or they're struggling with persistent shedding.

That's when the conversation becomes more interesting.

Instead of simply asking:

"What else can we add?"

I want to ask:

"Why isn't what you're already doing working?"

Sometimes the answer is inadequate intake.

Sometimes it's heavy menstrual bleeding.

Sometimes it's stress, hormones, thyroid function, or another medical issue.

And sometimes persistent digestive symptoms or nutritional deficiencies warrant a deeper conversation with a physician.

The goal of a comprehensive hair assessment is to recognize those patterns.

Signs Worth Discussing With Your Doctor

Hair loss alone doesn't mean you have malabsorption.

But consider speaking with your healthcare provider if hair changes occur alongside issues such as:

  • persistent diarrhea

  • chronic digestive discomfort

  • unexplained weight loss

  • recurrent nutrient deficiencies

  • difficulty maintaining ferritin

  • recurring B12 deficiency

  • significant fatigue

  • unexplained anemia

  • major changes in bowel habits

These symptoms require appropriate medical evaluation rather than self-treatment.

What Can You Do to Improve Nutrient Absorption?

There isn't one universal "gut protocol" that improves absorption for everyone.

The appropriate approach depends on the cause.

For someone with normal digestive function, the foundation remains fairly simple:

  • eat adequate protein

  • consume nutrient-dense whole foods

  • include healthy fats

  • eat a variety of plant foods

  • maintain adequate hydration

  • avoid unnecessary restrictive diets

  • address persistent gastrointestinal symptoms with a healthcare professional

If an underlying gastrointestinal condition exists, treating that condition is far more important than adding another supplement.

Bringing It All Together

Nutrition isn't simply about what goes into your mouth.

It's about what ultimately becomes available to your body.

Hair follicles rely on protein, iron, vitamins, minerals, fats, oxygen, and energy to continuously produce healthy hair.

If the digestive system isn't allowing those nutrients to be absorbed appropriately, the follicle may eventually feel the consequences.

Why is my ferritin still low even though I take iron?

There are many possible reasons, including inadequate dosage or intake, ongoing blood loss, heavy menstruation, increased physiological demands, or impaired absorption. Persistent low ferritin should be discussed with your healthcare provider.

Can poor absorption cause low B12?

Yes. Vitamin B12 absorption requires several normal gastrointestinal processes, and certain digestive or autoimmune conditions can interfere with them.

Will my hair grow back after correcting a nutritional deficiency?

If a nutritional deficiency is contributing to shedding and the follicles remain capable of healthy growth, correcting the deficiency may support recovery. Because of the hair growth cycle, visible improvement generally takes months rather than weeks.
